Once you have completed your portion of the form, please submit to Christina Pasana (christina.pasana@colostate.edu) along with applicable supporting documentation (e.g., course syllabus) for processing.

Use this form to request a change of concentration, change of campus, and/or addition of a second concentration.

Use this form to request substitution of a non-ColoradoSPH course for a general elective. Students must also submit Transfer Credit paperwork with an official transcript for all non-ColoradoSPH approved courses taken at a non-CU Denver institution.

Use this form to request an exception to a ColoradoSPH policy (e.g., number of home campus credits, years for degree completion, etc.).

Use this form to propose an Independent Study project for a specific semester. Only utilize this form after meeting with a faculty member who has agreed to serve as an Independent Study advisor. Students are not able to register for Independent Study credits until paperwork is submitted and approved. At this point, the student will receive a registration permission code from the CSU Graduate Advisor.

Use this from to request to 1) substitute an approved ColoardoSPH course for a required course or apply an approved ColoradoSPH course to a concentration-specific required area; 2) substitute a non-ColoradoSPH approved course for a required course or concentration-specific required area (in this case, students must also submit Transfer Credit paperwork along with a copy of their official transcript for all non-ColoradoSPH approved courses taken at a non-CU Denver institution).

Use this form to request to transfer credits from a non-CU Denver institution. Students must also send their official transcript with the course title and grade earned to the ColoradoSPH Office of Academic Affairs. For courses outside of the ColoradoSPH curriculum, students must also complete a ColoradoSPH Required Course/Concentration Required Area Substitution Request if they are hoping to apply the course toward their required core courses or a concentration required area.
